Has there always been monsters carrying offhand items? Last night I noticed that a sand elmental and a mergoyle had one in their hand. Maybe I just never noticed this before or this was something added in the update.

yea i noticed once i started sf farming, you can see what items they are holding, so if you see brohn stonehart with a brohn's staff, hes gonna drop it.

they don't always drop what you see and sometimes they drop more/opposite

wand instead of offand...staff instead of wand/offand combo

Yeah, a lot of times the monster will drop what they're carrying, like the Sparks of the Titans with their Magmas shields, but not always.
